do you need a transformer?

    No it cannot! It is nothing more than a circuit board that connects a USB jack to screw terminals.
    It is not clear what the purpose is. Connecting USB signals with this scheme would result in impedance mismatch and high error rate. In that regard, this is a particularly poor design. It may be intended to be used as a charging point, in which case a well-regulated 5.0 volt DC power supply would be required.
